How does one manage life with a narcissist who doesn't care about what the cost is or was to another person, but simply makes choices based on personal wants and needs leaving other people to live with the consequences? I'm exhausted . . .

One doesn't manage life with a narcissist, it's not possible unless the narcissist gets help. Narcissism leaves a wide path of destruction in it's wake. You should do yourself a loving favor and leave.
